A one-star Yelp post described an agonizing root canal that supposedly left the patient in pain for weeks. It claimed I had ignored an infection and sent them home with nothing but ibuprofen. The reviewer said my staff laughed at them in the waiting room. There were details that sounded plausible, a back molar, a late Friday appointment, a swollen jaw. The account had no photo and only a single review, this one. It had been posted within an hour of an unrelated one-star review left on a med spa two states away. The writing style in both was oddly similar, down to the same misspelling of "anesthetic."

What actually happened

I keep meticulous scheduling records, and there was no Friday root canal matching that description in my books. The infection story does not correspond to any chart I can find. Yet I cannot say any of this publicly, because acknowledging or denying the appointment would breach patient confidentiality. The maddening part of HIPAA is that it protects the very person who may be lying about me. I am not permitted to say "this event did not occur in my office" if doing so implies knowledge of a specific person's care. My attorney was clear that even a careful denial could be construed as a disclosure. So the accusation stands, unanswered, in a way that looks like an admission.

Impact

The review dragged my Yelp average down just as I was trying to grow the practice. Two prospective patients told my receptionist they had chosen another office after reading it. I started dreading opening the app, which I had once checked with pride. There were nights I lay awake composing responses I knew I could never send. My associate noticed I had become short-tempered and asked if everything was alright. The emotional weight of being called cruel to a patient, when kindness is the core of my work, was heavier than I expected. It made me question whether the profession's public rating systems were fair to anyone.

Steps taken

I reported the review to Yelp as fraudulent and provided the timeline showing a duplicate elsewhere. Yelp's response was that the review did not clearly violate their content guidelines. I appealed twice and received the same boilerplate reply each time. Eventually the review was hidden behind Yelp's "not recommended" filter, though it still counted against me in visible ways. I spoke with a lawyer about a defamation claim but learned the anonymous account made it nearly impossible to pursue. She advised documenting everything and posting a single neutral statement about my commitment to care. In the end I did nothing publicly, because doing something safely was not possible.
